If your membership includes a visit fee ($25- telehealth, $50-office, $75- home visit within Dane County), you'll have the option to select a payment method below. For home visits outside of Dane County, please contact Dr. Giselle for pricing.
If nothing is selected or you no-show your appointment, the fee will be added to your next invoice.
